The clinical dividing line
Nursing registration means qualified nurses are on the premises at all hours, and that determines which needs a home can safely accept. Complex wounds, catheters, feeding tubes, conditions requiring regular observation and care through the final stages of an illness all sit inside it. Everything a residential home provides remains alongside: a private room, meals, laundry, help with washing and dressing, and company. Thirty one homes here hold this registration, so the task is narrowing a strong field rather than finding one at all.
What the ratings do and do not tell you
Four homes carry Outstanding judgements, among them The Links, Canford Chase and Queensmount Care Home, with twenty two rated Good, two at Requires Improvement and three not yet rated. Start with the date of each inspection, since a judgement made some years ago may describe a home that has changed hands or managers since. Then read what was recorded about staffing, medicines management and how residents spend their days, because homes sharing a rating often read quite differently.
Nursing cover is what to press on
Ask how many registered nurses are rostered by day and overnight, and how much of that cover comes from agency staff rather than the home's own team. Ask how falls, pressure areas, chest infections and support at the end of life are managed, and how quickly a GP will attend. Ask how long the manager has been in post and what turnover among nurses has been, since a home that cannot retain nurses struggles with everything depending on them.
Getting an assessment and sorting out the funding
The needs assessment
An assessment of need is free, available to anyone who appears to require care and unaffected by savings. It examines mobility, continence, medication, nutrition, memory and the nursing input required, and produces a care and support plan setting out what a home must be able to deliver. Ask Bournemouth, Christchurch and Poole Council adult social care to arrange one, since it is responsible for assessments and for agreeing any funded support that follows. Relatives providing unpaid care can be assessed in their own right.
Three routes to funding
The council route rests on a financial assessment of income, savings and capital, and in a permanent placement the value of a property is normally included unless a partner or certain other relatives still live there. Because nursing is involved, NHS Funded Nursing Care contributes towards the nursing element and is paid directly to the home, while NHS Continuing Healthcare can meet the whole cost where needs are primarily health related rather than social. Each is assessed separately, so raise all three by name rather than waiting to be offered them.
Fees and the longer view
Establish what the weekly charge includes and what is billed on top, whether chiropody, hairdressing, continence products or escorted appointments. Ask how often fees are reviewed and what the last increase amounted to. Ask what the home does when a self funding resident's capital runs down and council support becomes necessary, because that answer decides whether another move could be forced at a difficult moment.
