What should I ask when visiting a nursing home in Oxfordshire?

Ask how many registered nurses are on each shift and how the home covers absence at short notice. Find out how long the manager has been in post and how much of the care team is permanent rather than agency. Ask how the home works with local GPs and what happens if someone becomes unwell overnight. It is also reasonable to ask what the last inspection found and what has changed as a result.

How does a nursing home differ from a residential care home?

A residential care home provides accommodation, meals and personal care, with community nursing called in when clinical care is needed. A nursing home is registered to have a qualified nurse on duty at all times, so treatment can be given on site. The homes on this page are registered as nursing homes. If needs are modest now but expected to increase, ask whether a home is registered for both, because that can avoid a second move later.

Will Oxfordshire County Council fund a nursing home placement?

Oxfordshire County Council can fund all or part of a nursing placement once a care needs assessment and a financial assessment have been completed. What it pays depends on assessed need and on capital and income, with national thresholds applied to savings and property. Where needs are primarily health related the NHS may fund the placement instead, and a separate contribution towards nursing costs applies where a registered nurse is required. Ask for written confirmation of what each body will pay.

How do I arrange a care needs assessment in Oxfordshire?

Contact the adult social care team at Oxfordshire County Council and request a care needs assessment, which anyone who appears to need support may have regardless of income. A practitioner will look at what your relative can manage safely and what level of care is required. A financial assessment then establishes whether the council will contribute towards the cost. Ask early, because assessments take time to arrange and the written outcome is useful evidence when approaching homes.

How far ahead should we start looking?

Begin as soon as nursing care looks likely rather than waiting for a crisis, because homes with a strong reputation rarely have an immediate vacancy. Visiting two or three while there is no pressure makes the differences between them much clearer. If a hospital discharge is setting the pace you may have only days, so ask the ward social work team for help and treat the first placement as reviewable. What nursing care in Oxfordshire involves and what to look for

A nursing home differs from a residential home in one decisive respect: a registered nurse is on duty at all times, so clinical care can be given on site rather than waiting for a visit. That suits someone with a long term condition, complex medication, wounds needing regular attention, or health that is changing week by week. Be specific about the nursing actually required at the first conversation, because not every home takes on every type of clinical need.

Staffing is the thing to examine

Ask how many registered nurses are on duty across a full day and night and how the home covers a nurse who calls in sick. Ask how it works with local GP practices, what happens when someone deteriorates overnight, and how quickly families are told. Watch how staff speak to residents who cannot easily speak back, and whether people are helped to change position and take a drink without having to ask. Ask how the home records preferences about routine, food and religious observance, and who makes sure they are respected.

What the inspection reports show

All seventy eight homes are registered with the Care Quality Commission. Fifty seven are rated Good and nine are rated Outstanding, seven are rated Requires Improvement and five are not yet rated. In a nursing setting the detail matters more than the headline, so read what inspectors said about safety, medicines and whether the service is well led, and note when the visit happened. A home told to improve that has since replaced its management is a different proposition from one with the same grade and no changes at all.

Getting an assessment and funding a nursing placement in Oxfordshire

The care needs assessment

An assessment is free and open to anyone who appears to need care and support, whatever their savings. Oxfordshire County Council is responsible for carrying it out and for deciding what support it will fund, so its adult social care team is the right first contact. Where nursing is involved the assessment should establish whether needs are primarily health related, because that affects who pays. A financial assessment then looks at income, savings and property to work out any contribution.

Fees, funded nursing care and top ups

Nursing fees are generally higher than residential fees and vary with the room and the level of clinical care, so ask for the current weekly figure in writing and for a list of anything charged separately. Where a resident is assessed as needing care from a registered nurse, the NHS pays a contribution towards that nursing element directly to the home, and the home should show you how it appears on the account. Where the council funds a placement it pays up to the amount it considers appropriate for assessed needs, and a third party may be asked to top up for a more expensive home. Check the notice period and what happens to fees during a hospital admission before signing.
