About PRC Outreach

PRC Outreach provides care and support in people's own homes and in the community across Penzance and the surrounding parts of Cornwall. We work with older people and with younger adults, including adults who have learning disabilities, autistic adults, people with physical disabilities and people living with sight or hearing loss. Support may cover personal care, help with medication, meals, shopping and household tasks, and often the wider business of living as well, from appointments and paperwork to getting out and seeing people. Support is planned with the person rather than for them. We begin by asking what somebody wants their week to look like, what they can already manage and where help genuinely makes a difference, and we write the plan from those answers. It changes when circumstances shift or when the person changes their mind. We support people living with dementia and keep the same carers with the same people as far as the rota allows, because familiarity settles people in a way that nothing else does. For autistic people we pay close attention to communication, predictability and sensory needs. The Care Quality Commission rated us good at our most recent inspection.
