What nursing care in Wirral involves and what to look for
Qualified nursing on the premises at every hour is what separates these homes from residential ones, and it is the capability a family is paying for. On a list where the published record varies as widely as this one does, it is also the thing to examine hardest before committing.
Matching the home to the clinical need
Ask which needs the home supports as a matter of routine rather than occasionally. Pressure wounds, feeding tubes, catheter care, advanced dementia, Parkinson's and end of life care each demand different training and different habits, and competence in one does not imply competence in another. Ask how many residents it currently supports with comparable needs, and ask what would happen if the person's condition changed markedly, because the answer decides whether this placement is likely to hold.
Taking the mixed ratings seriously
Twenty six homes here hold a Good rating, so there is a workable group to start from. Fourteen sit below it, and those judgements are not interchangeable: open each report and look at which of the regulator's key questions was marked down, since a finding on governance or record keeping is a different matter from one concerning safety, dignity or medicines. Ask the home directly what has changed since, and whether the registered manager is the same person. Three homes carry no rating, usually after a recent registration, and there your own visit has to do the work a report normally would.
Nursing cover is the question behind the question
Ask how many registered nurses are on duty by day and overnight, and how a shift is covered when one is absent. Ask what proportion of nursing shifts is filled by agency staff, because a settled team spots deterioration sooner than a rotating one. Ask how the home works with visiting general practitioners, district nurses and specialist teams, and how quickly a doctor is called when someone becomes unwell at night.
Getting an assessment and arranging funding
The care needs assessment
A care needs assessment is free and available to anyone who appears to need care and support, whatever their savings. The adult social care team at Wirral Metropolitan Borough Council carries it out. Its most consequential finding is whether nursing care is required or whether a residential home would meet the need, because the two differ in cost, in availability and in the funding attached to them. It also records what help is needed with washing, dressing, medication, mobility and overnight support. Ask for the outcome in writing before approaching homes.
Three routes to paying the fee
The council's financial assessment sets income and capital against the national threshold and fixes its contribution, with property treated according to who else lives in it. Where someone is assessed as needing care from a registered nurse, the health service may pay towards the nursing element of the fee, and where health needs are substantial and ongoing, continuing healthcare may cover the whole package instead. Both are health service decisions, so ask plainly whether either assessment has been done. On fees, get in writing what the weekly figure covers, what is billed on top, and how often it is reviewed.
