Arranging an assessment and settling how nursing care in Tunbridge Wells is funded
Why the funding question comes first
Nursing placements carry the highest fees in social care, and the help available comes from two directions rather than one, so leaving the question until a home has been chosen rarely works well. The starting point is a care needs assessment, which Kent County Council has a duty to provide to anyone who appears to need care and support. It is free, it is not means tested, and a relative may request it on someone's behalf, as can a GP, a district nurse or a hospital discharge team. The written outcome records which needs are eligible and is worth holding even where a family expects to fund the placement itself.
The council, the health service and the means test
A financial assessment follows, setting capital and income against the national thresholds. Below the upper capital limit the council contributes towards the fee while the resident pays an assessed amount from income and savings; above it the placement is self funded, although the council can still be asked to arrange it. Separately, anyone in a nursing home who is assessed as needing care from a registered nurse qualifies for NHS funded nursing care, a flat weekly contribution paid towards the nursing element regardless of capital. Where the primary need is a health need rather than a social care one, NHS continuing healthcare may cover the whole placement, and it is worth asking for that assessment explicitly rather than waiting to be offered it.
What to establish in writing
Ask every home for a written schedule of fees showing what the weekly figure covers and what is charged on top, whether that is chiropody, hairdressing, escorted appointments or continence products beyond the prescribed allowance. Check whether the NHS funded nursing care contribution is deducted from the fee or simply absorbed by it, because the two are not the same for the family paying. Confirm what happens should savings fall to the threshold during the stay, what notice either side must give, and whether the home accepts council funded residents at all.
Choosing between the nursing homes in Tunbridge Wells
What a nursing registration actually provides
All twelve providers here are registered as nursing homes, meaning a qualified nurse is on shift through the night as well as the day. That matters for wound and pressure area care, for medication given by injection, for assisted feeding, for managing catheters and stomas, and for recognising deterioration early enough to act on it. If the person needing care has stable needs and chiefly requires help with washing, dressing and daily routine, a residential home may suit just as well at a lower cost, so it is worth being honest about which of the two the clinical picture really calls for.
Reading the ratings in this set
Eleven of these homes are rated Good by the Care Quality Commission and one has been told it requires improvement. That consistency means the ratings alone will not do much to separate the field, so the reports themselves carry the work. Each contains findings on whether a home is safe, effective, caring, responsive and well led, and for nursing care the safe and effective sections repay close attention, since they cover staffing levels, medicines management and clinical oversight. Note the inspection date too, because a judgement made some years ago may describe a home since changed in ownership or management.
What to look for on the visit
Ask how many registered nurses are on duty at night and how often agency staff fill those shifts, as continuity of clinical judgement is harder to replace than any other kind. Ask who reviews care plans, how often, and how the home works with the local GP practice and district nursing team. Notice whether residents who cannot call out are being checked on, whether people are repositioned without being hurried, and whether the atmosphere feels clinical or lived in. Ask plainly what the home does when a resident deteriorates at the weekend.
