Arranging nursing care in Newcastle upon Tyne and meeting the cost
Nursing placements carry an extra layer that residential ones do not, because the NHS may be responsible for part or all of the cost. Sorting out who pays before choosing a home avoids a good deal of difficulty later.
The care needs assessment
Anyone who appears to need care and support is entitled to an assessment, whatever their savings, and for nursing care it also establishes whether a nursing registration is genuinely required. The assessment covers washing, dressing, mobility, medication, clinical needs, safety and the support a relative is already giving, and it produces a written record. Requests are made to Newcastle upon Tyne City Council, which is responsible for adult social care assessments and for arranging funded support in the city. A relative providing unpaid care can request a carer's assessment of their own at the same time.
Fees, means testing and NHS contributions
A financial assessment follows, taking account of income, savings and, for a permanent move, the value of a property in most circumstances. Capital above the threshold means the resident pays; below it the council contributes according to the assessment. Where a place in a nursing home is needed, the NHS may pay a contribution towards the nursing element of the fee, and where the main need is a health need, NHS continuing healthcare can cover the cost in full. That is assessed separately, so ask about it early rather than assuming it has been considered. Fees otherwise are set by each home, so they vary across Newcastle upon Tyne with the room, the level of nursing input and the complexity of the care, and every home should provide a written breakdown of what the weekly charge includes and what sits outside it.
Choosing a nursing home and what to look for
Working with the inspection reports
The Care Quality Commission judges each home on five questions, asking whether it is safe, effective, caring, responsive and well led, with the overall rating summarising those five. Five homes here are rated Outstanding and twenty four Good, while three sit at Requires Improvement, where the important question is what specifically concerned inspectors and whether a revisit has happened. Eight homes have no rating yet, usually because they registered recently. For nursing care, pay particular attention to what reports say about medicines management and staffing, since those are where clinical risk tends to show.
Questions specific to nursing care
Ask how many registered nurses are on duty across the day and overnight, and how shifts are covered when a nurse is unwell. Ask how the home works with district nurses, GPs and hospital teams, and how quickly a deterioration would be picked up and acted on. Ask what the home does for people whose needs become palliative, and whether someone would be able to stay rather than move again. Visit twice if you can, with one visit around a mealtime, and watch how staff speak with residents who are not part of your tour.
