About Council Reablement Service (Hub)

We are the Wiltshire Council reablement team working out of the Salisbury hub, supporting people in their own homes across the city and the surrounding area. Reablement is short term by design. We come in at the point where somebody has come out of hospital, or has had a fall or an illness that knocked their confidence, and the aim is to help them get back to doing things for themselves rather than to take those things over. That means we often do less as the weeks go on, which is exactly what success looks like in our line of work. We support older people and younger adults, people living with dementia or a mental health condition, people with a learning disability or autism, people with a physical disability, people with a visual or hearing impairment, and people dealing with alcohol or drug misuse. Support covers personal care, medication, meals, moving about safely at home, and building back the everyday routines that illness interrupts. Where somebody will need longer term support once our work is done, we say so early and help arrange it, so that nobody is left without a plan when we step away. Our most recent inspection by the Care Quality Commission rated us good overall.
