About Us
We are the adult community care team at Guy's and St Thomas' NHS Foundation Trust, working with people in their own homes across Southwark. Our support sits alongside the wider NHS care that people receive, and it is arranged around the life somebody already has rather than around what is convenient for us.
We work with older people who need help with daily living, and with younger adults whose health or disability means that practical support at home makes the difference between managing and not managing. What that looks like varies a great deal. For some it is help with personal care, medication or meals. For others it is support that allows somebody to come home after a hospital stay rather than remain there longer than they need to, or assistance that keeps a condition from escalating into another admission.
Because we are part of a wider trust, we can join up what happens at home with what happens elsewhere in a person's care, and we can raise a concern with colleagues quickly when something changes. Our staff are encouraged to notice and report the small shifts in appetite, mobility or mood that often come before a more serious problem.
We start with a proper assessment and an honest conversation about what would genuinely help, and we review arrangements as needs change.
