About BPJ Case Management
BPJ Case Management supports people with a physical disability in their own homes in Middlesbrough and the surrounding area. Our work is with people whose needs are complex enough that the support has to be organised properly as well as delivered well, and we take on both sides of that. Support at home can mean help with personal care, with moving safely, with equipment, with medication and with the practical running of a household. Just as important is the coordination: making sure the therapists, nurses, doctors and everybody else involved are working to the same plan rather than in separate directions, and that the person at the centre of it is the one making the decisions. We start by getting to know somebody properly, including what they are trying to get on with, what they can do for themselves and where they genuinely want help. Plans are made with the person, and reviewed as things change rather than left to go stale. Our staff are trained for the individuals they support, including the particular skills a person's needs call for, and we keep teams as consistent as we can, because support this personal depends on trust that is built over months. If you would like to know more about how we work, please get in touch and we will explain it plainly.
