About Hammersmith

Hammersmith provides care and support in people's own homes across Kensington and Chelsea and the neighbouring parts of west London. We work with older people, adults living with dementia or mental health conditions, adults with learning disabilities and autistic adults, people with physical disabilities, people with sight or hearing loss, and younger adults living independently. Our most recent inspection by the Care Quality Commission rated us good overall. Home care only works when it is reliable. If a visit is late, someone sits waiting, medication slips and confidence drains away. So we plan rounds carefully, keep them geographically sensible and arrange proper cover for sickness and holidays rather than leaving people to hope. What the visits cover is agreed with the person. It might be help with washing and dressing, medication, meals and shopping, or support to get out, keep appointments and stay connected to the people and places that matter. For some of those we support, particularly younger adults and people with learning disabilities, our role is more about building confidence than doing things for them. London is home to people from many backgrounds, and we ask about language, faith and food at the start so that we can match carers thoughtfully. If you would like an honest conversation about what support would help, please get in touch and we will arrange to visit you.
