About Us
We are Care By Us at Swan Field Court in Welwyn, Hertfordshire, providing care and support to people in their own homes. We work with older people and with younger adults who need help to live independently.
Our starting point is always the person's own routine. Care at home should fit around the life somebody already has, not rearrange it, so we begin with a visit and a proper conversation about what help is wanted, when it would be most useful and what the person would rather manage themselves. From that we agree a plan, and we change it as often as circumstances require.
Support might include help with getting up and going to bed, washing and dressing, prompting or assisting with medication, preparing meals, keeping on top of the housework, or getting out to appointments, shops and social occasions. For some people the most valuable part of a visit is the company and the chance to talk to somebody who is genuinely interested.
We try to keep the same carers going to the same people, because trust takes time and because a familiar carer notices when something is not right.
Families are kept informed with the agreement of the person we support.
