About Us
Medical Partnerships supports people in their own homes across Rayleigh and the surrounding parts of Essex. We work with older people and younger adults, people living with dementia, people with a learning disability or autism, people with a physical disability, and people with a visual or hearing impairment.
The point of care at home is that life carries on where it already is. Nobody has to pack up a house, leave a garden or learn a new neighbourhood. Our job is to fit around the household rather than reorganise it, so we begin by asking how things already run and build the support to match.
Visits cover personal care, support with medication, meals and drinks, help around the house and with shopping, and support to get out to appointments or simply for a change of air. Some people want half an hour each morning. Others need several calls a day, and the right amount often changes over time.
Where sight or hearing has become restricted, small practical adjustments make a real difference to how independent somebody feels, and we treat that as part of the job.
Our most recent inspection by the Care Quality Commission rated us good overall.
