Our job is to make that sensible rather than to take anything over. Depending on the person that could be a single morning call to help with washing and dressing, several visits a day covering personal care, medication, meals and company, or a longer sit so a family carer can get out for a few hours. We begin by asking what help is actually wanted, which is not always what a referral form says. People are usually clear about what they can still manage and would rather keep doing, and respecting that is as much the job as anything we take on. Where somebody has a physical disability, we think properly about equipment, moving and handling, and how much help to offer, because being helped more than you want is its own kind of loss. Continuity is where we put our effort. A familiar carer who knows the house, the routine and the person makes a visit work in a way nobody new can, and that is most true where dementia is involved.
