About Heart2Heart.

Heart2Heart provides care and support to people in their own homes across Southport and the wider Sefton area. We work with older people and younger adults, with people living with dementia, with people managing mental health conditions or eating disorders, with people affected by alcohol or drug misuse, and with people who have physical disabilities or a visual or hearing impairment. That is a wide range, and it reflects the fact that people's lives rarely fall into a single category. Somebody may be managing a long term health condition while also coming through a difficult period with alcohol, or living with a sensory impairment alongside a mental health condition. We try to look at the whole situation rather than the referral heading, and we plan support with the person rather than for them. Our carers help with personal care, medication, meals, household tasks and getting out into the community. Often the most valuable part is simply reliability, because turning up when we said we would, to somebody who has had plenty of people not turn up, does more than any individual task on a plan.
