Here2Care is a home care agency in Dartford, supporting people across the surrounding parts of Kent in their own homes. Most of the people we visit want to carry on living where they are, surrounded by their own things, and our role is to make that practical rather than to take over. We support older people, people living with dementia, adults with a learning disability, autistic adults, people with a physical disability and younger adults. Visits range from a short morning call to help someone wash, dress and get the day started, through to several visits a day covering personal care, medication, meals and company. Some people need us for a few weeks while they recover. Others have been with us a long time. Consistency is the thing families ask us about most, and rightly so. Having a small, familiar group of carers rather than a different face each time makes a real difference, particularly where dementia is involved, and we plan rounds with that in mind. We also try to keep visit times steady, because a call that drifts by an hour can unsettle an entire day. Care plans are written with the person and, where they want it, with their family. We review them properly when things change rather than leaving them to go stale, and we will say plainly if we think someone needs more support than we are providing.
