We agree what is wanted with the person rather than handing over a standard package, and we review arrangements when circumstances change instead of leaving them as they were set up months ago. If somebody needs more from us, or if what we provide is no longer keeping them safe at home, we will say so rather than let things drift. For people living with dementia, knowing the person makes all the difference, so we ask families to tell us about the life somebody has led and we use that rather than filing it away. We keep the same carers with the same person wherever the rota allows, because familiarity makes personal care easier and because a familiar carer spots early when things are wrong.
