About Transform

We are Transform, a home care agency working with people across Greenwich and the surrounding parts of London. Our support happens where it matters most, in a person's own home, among their own belongings, routines and neighbours, and we build our visits around the shape of somebody's ordinary week rather than asking them to fit around us. Much of our work is with older people who want to carry on living independently with some practical help, whether that means assistance with washing and dressing, medication, meals, or simply someone reliable arriving at the same time each day. We also support people living with dementia, where a calm and unhurried approach makes a real difference, and we take time to learn a person's history so our visits feel familiar rather than intrusive. Alongside this we support adults with a learning disability or autism, people managing their mental health, and people working through difficulties with alcohol or drug misuse. That kind of support asks for patience and consistency, so we do our best to send the same faces and to keep our promises about timing. We treat that as a starting point rather than a finish, and we keep asking the people we support and their families what is working and what needs to change.
