About Us
The Joint Community Rehabilitation service supports people at home in Lewes and the surrounding parts of East Sussex, usually for a short period and usually at a point of change. Much of our work follows a hospital stay, a fall or a spell of illness, and the aim is to help somebody get back to managing for themselves rather than to take over.
That means we work towards doing less over time, not more. We help with personal care, medication, meals and the practical running of a home while a person rebuilds confidence, and we encourage them to take things back on at a pace that is safe. Where longer term support is needed we say so and help arrange it.
We support older people, people living with dementia, adults with mental health needs, those with a physical disability and people living with sight or hearing loss. We also work with people living with eating disorders and people affected by alcohol or drug dependency, always alongside the health professionals involved.
We work closely with therapists, district nurses and doctors, because short term support only achieves anything if everybody is pulling in the same direction.
Please get in touch if you would like to know more about what we do and how support is arranged.
