About Farm Lane
We provide nursing care at a home on Farm Lane in Hammersmith and Fulham, for people whose needs require qualified nurses alongside support with daily living. We care for older people, those living with dementia, adults with learning disabilities, autistic people, people living with mental health conditions and people with physical disabilities. Work of this kind cannot run on routine alone. It takes nurses on duty so that medication, pain, wounds, continence and difficulties with eating, drinking and moving are managed properly and without waiting, and it takes staff who know each person well enough to see a change before it becomes a crisis. Where somebody is autistic or has a learning disability, the way care is offered often matters more than what is offered, so we pay attention to how a person communicates, what they find intolerable and what helps them feel safe. Around all of that we try to protect ordinary life: choosing when to get up, what to wear, what to eat and who you want beside you. For people living with dementia, patience and familiarity settle somebody far better than any procedure. We work closely with general practitioners, community teams, therapists and hospital staff and keep families properly informed. Respite stays are available as well as longer places. Our most recent inspection by the Care Quality Commission rated us good overall. Please come and see us and ask whatever you need to.
