About Us
We are Leek Moorlands Hospital, and the support described here is the care we provide to people in their own homes in Leek and the surrounding part of Staffordshire. Our purpose is to help people manage at home, with the right support arriving reliably, rather than having to move somewhere else to get it.
We work with older people and younger adults, with people living with dementia and with people who have physical disabilities. A visit may involve help to wash and dress, support with medication, preparing meals, assistance with mobility and moving safely about the home, light household tasks, or company and support to get out to appointments.
Support is agreed with the person before anything begins. We ask what time they like to get up, what they want to eat, what they want help with and what they would rather manage themselves, and we involve family and other professionals as far as the person wishes. The plan goes in writing and is reviewed when circumstances change. We try to keep the same carers calling on the same people, because familiarity makes support easier to accept, particularly for someone living with dementia.
