About AJ Case Management

We provide support in people's own homes across Shropshire and the surrounding area, working with older people, with people who have a physical disability, with people whose sight or hearing is impaired and with younger adults. A good deal of our work involves people with substantial and long standing needs, often following injury or illness, where support at home has to be planned carefully and coordinated with everybody else involved. We work alongside therapists, clinicians, equipment providers and families so that arrangements fit together rather than pulling in different directions. Where somebody has a physical disability we take the time that moving and transfers properly require, using the right equipment and explaining as we go. Where sight or hearing is impaired, consistency is everything. Belongings stay exactly where people expect them, we say who we are before we begin and we check we have been understood. Plans are written with the person, in plain language, and reviewed openly as circumstances change. We would far rather be told plainly that something is not working than find out months later.
