About Your Choice Your Future

We are Your Choice Your Future, based at Much Wenlock and supporting people in their own homes across Shropshire. We work with older people and people living with dementia, adults with a learning disability and autistic adults, people managing a mental health condition, people with a physical disability or a visual or hearing impairment, and younger adults who need support to live independently. The name is meant literally. Support is planned with the person, around the life they want, rather than around what is convenient for us to deliver. For some that means help with personal care, medication, meals and keeping a household running. For others it is backing to hold down a job or a course, to keep a tenancy going, or to do the ordinary things without somebody else taking charge. Working across a rural county means being honest about distance. We plan rounds around real travel times so that visits are not cut short, and for people living some way from the nearest town our worker may be the main contact of the day. We keep the same workers with the same households wherever we can, and staff are trained around the individuals they support rather than in the abstract. Please get in touch to discuss support.
