We are Pembroke Care (Reading), a home care agency supporting people in their own homes across the town and the surrounding area. We work with older people and younger adults, including those living with dementia, people managing a mental health condition, people with a physical disability and people with a visual or hearing impairment. Our starting point is the life somebody already has rather than a list of what we could provide. What does an ordinary day look like, what has become hard work, and what does the person want to keep managing themselves. The visits are built from those answers and reviewed as circumstances change. Continuity is where most of our effort goes, and it is harder to organise than people assume.
