We start with a visit at home, because a house tells you a great deal that no assessment form will. What we want to establish is where help would make a real difference and what the person would rather keep managing themselves. The plan comes out of that conversation, and it belongs to the person to change. Support might be a single daily call for personal care and medication, or several visits covering meals, housework, shopping, company and getting out and about. Reliability is what decides whether care at home works. Arriving at the time agreed. Staying the full visit rather than rushing off. Sending carers the person already knows. Ringing a family before they have to ring us. We keep rounds local so that those are promises rather than intentions. Where somebody is living with dementia we pay close attention to timing and familiar faces. Where there is sensory loss we think about lighting, contrast and hearing aids that have actually been checked, which are small things that decide whether a person is part of a conversation. Our carers are checked and trained before they visit anybody, and we are registered with and regulated by the Care Quality Commission.
