About Us
Liaison Care provides support in people's own homes in Chesterfield and the surrounding parts of Derbyshire. We work with older people and with younger adults who need a hand to carry on living independently.
We think the point of home care is to fit quietly into someone's existing life rather than to reorganise it. So our first conversation is about what has become difficult and what the person wants to keep doing for themselves. From there we agree what our visits will cover, which might be help to wash and dress, support with medication, meals, laundry and housework, shopping, or company and a check that all is well.
We write the plan down in plain language, we share it with the person and with family where they want that, and we change it when circumstances change rather than leaving it to drift.
Continuity is something we take seriously. Letting someone into your home to help with personal care requires trust, and trust is built by seeing the same faces. We keep teams small, we turn up when we say we will, and we tell people promptly if something has to change.
Our carers are also our early warning system. Someone who visits regularly notices when a person is eating less, sleeping badly or seeming unsteady, and we act on those observations rather than filing them away.
