From that conversation we agree a plan, with family or an advocate involved whenever somebody wants them there, and we revise it as circumstances change rather than at fixed review dates. Slough is a town of many communities and many languages, and we try to match support workers to households in a way that takes account of language, food, faith and custom as well as care needs. Visits cover personal care, medication, meals and drinks, housework and laundry, shopping, getting to appointments, and company. For people living with dementia we keep the same small team visiting and work at their pace. Where somebody is managing a mental health condition we keep support consistent and work alongside their community team.
