About Us

Meridian Health and Social Care in Bradford supports people in their own homes across the district. We work with older people and younger adults, including people living with dementia, people with a learning disability or autism, people managing mental health conditions, and people with a physical disability or a visual or hearing impairment.

Because the people we support are so different from one another, we plan individually rather than offering set packages. We meet the person and anyone they want involved, work out what would genuinely help, and write it down in plain language so that every carer who visits is working to the same understanding.

Support might be personal care, medication and meals, or it might be broader, covering household tasks, shopping, appointments, getting out of the house, or holding a routine together that has become hard to manage alone.

We think carefully about who we send. Language, cultural background, gender and simple personality all matter in someone's own home, and we take them into account when we build a rota. We aim for small, consistent teams and for turning up when we said we would.

Our most recent overall rating from the Care Quality Commission is good. With the person's agreement we keep families informed, and we raise concerns early with health professionals.
