About Us

We are the reablement and home first team for Blackburn with Darwen, supporting people in their own homes at the point where things have changed, often after a spell in hospital or a sudden loss of confidence. Our work usually runs for a set period and has a clear purpose, which is to help somebody get back to doing as much for themselves as they can rather than settling into long term support they may not need.

We work with older people, people living with dementia, adults with a learning disability and autistic adults, people managing mental health conditions, people living with a physical disability and people with sight or hearing loss. We also support people living with eating disorders, people in recovery from alcohol or drug misuse and younger adults.

Reablement only works if it is honest. Sometimes somebody regains far more than anybody expected, and sometimes the right answer is that ongoing care is needed, and we say so plainly.

Our most recent overall rating from the Care Quality Commission was requires improvement. We would rather state that ourselves. We have taken the findings seriously and our focus has been on reliability and on communication with the people we support and their families.
