We start with the person rather than the diagnosis, because two people with the same condition often need entirely different things. A first conversation, with whoever the person wants present, tells us far more than any form, and from it we agree a pattern of visits that is honest about what we can keep to. Support can include personal care, medication, cooking, laundry, shopping, and getting out to appointments, work or study. Supporting somebody around food and eating asks for particular care and a complete absence of pressure or comment, and we work to whatever plan is already in place with the clinicians involved rather than inventing our own. Continuity takes real effort in a busy part of London. We plan rotas so people see faces they know, and we brief anybody new before they arrive.
